Project in POC phase

How are you, really?

Sofia.care helps clinicians understand how a patient is doing between two appointments.

A short, regular, caring exchange keeps a trace of what happens between consultations, so the patient feels less alone and the clinician walks into the session with real context.

Let's talk

Looking for first hospital partners in Wallonia.

The need

Between two consultations, a lot of information gets lost.

In mental health, a large part of what matters happens far from the clinic: the highs, the lows, the breakpoints, the weak signals, the progress that often goes unseen.

At the next appointment, clinicians often have to reconstruct days or weeks in just a few minutes.

Sofia.care starts from a simple reality: to help better, it sometimes takes better understanding of what happened between two visits.

The solution

A bridge between the patient and the care.

Sofia.care offers a short, regular, caring space for dialogue.

Patients can drop in how they feel, without judgement. Their answers are then structured over time to surface trends, shifts, and important moments.

The goal isn't to add load on the clinician, but to give them a clear, useful summary right when the consultation starts.

Scope of use

A support tool, not a diagnostic one.

Sofia.care does not replace doctors, psychologists or psychiatrists.

  • No diagnosis : The project makes no diagnosis and takes no medical decision.

  • Clinician supervision : Sofia.care operates inside a framework agreed with the referring professional, never as an autonomous tool.

  • Secure data : European hosting, data encryption, and no commercial use of patient data.

  • Formal ethical framework : The POC will be reviewed by an ethics committee, co-built with partner teams.

Become a partner

Let’s build the first POCs from the field.

Sofia.care is preparing its first POCs. We want to collaborate with hospitals, mental health services, doctors and clinical teams to test the concept in a secure, ethical and useful frame.

The goal is to validate the project’s usefulness, its acceptability by patients, and its added value for care teams.

You work in a hospital, mental health service or care structure? Let’s take the time to talk.
